What Happened in the Time Before Chapter One?

The cover of this book is beautiful and intriguing, almost otherworldly - and the title has me curious as well. Reading the first chapters thrusts the reader into Cohen's life in a dramatic way ... what occurred in the minutes and hours before? Who can be trusted? I thoroughly enjoyed the flashbacks to Cohen's memories of childhood at church.
